How to become a New Patient?

Fill out our New Patient Form and submit to our check-in desk. Once we have reviewed the form we will contact you to schedule your New Patient appointment.

How to Refill Medication?

If registered with the Healow App, follow these instructions or watch this "How to video"

  1. Navigate to the dial on your homepage

  2. Click Medication

  3. Click the plus (+) icon in the upper right hand corner, and select "Request Refill" on the bottom prompt.

  4. Select the medications needed 

  5. Enter your preferred pharmacy and any additional notes 

  6. Submit your request 

If not, call our office and your requested will be processed within 48 hours. 

*Note: Controlled substances may be subject to varying wait time due to patient eligibility process.

How to view Lab Results?

If registered for Healow, Select the lab Icon on the dial located on the home page. Provider notes are able to be viewed at the bottom of the page. 

If not registered, come by our office a request a print out at our check-in.

How to schedule a COVID -19 Test?

Call our office to schedule a test or request one by messaging us on the Healow app.

We offer: 

Care Start Rapid Test ( 20 Minutes ) 

PCR COVID Test ( 2 Business Days)

How to pay a balance?

Paying you balance can be easily done by visiting the link on your mailed statement. 

If you do not have a link, you can log in to the patient portal or call us at

(910-426-3332).

I have submitted paperwork to be filled out, when can I expect it?

Generally paper work will be filled out within 2 weeks. Depending on the type of form, some will take longer to complete

How do I request my medical records?

Medical records request can be completed by messaging our office and specifying whether request is for personal use or transfer to another providers office 

Personal Request:

  • CD- $6.50 

  • Paper copy: Price will vary depending on page number

 

Provider office request:

There will not be a charge associated with request if the secondary provider is requesting from their office

No Show Policy/Cancelation Policy 

Our office will contact you to remind you of your upcoming appointment. If you are 15 minutes late, you will forfeit your appointment slot and be charged 15$. Payment must be made prior to scheduling another appointment or blood test.
